Co-Designer Media Release form: Required for co-designer personal information (e.g. name, photograph, video) to appear on team-submitted materials.

Team Registration form: To be filled out by team coaches (high school) or captains (college). Please note that you must be 18 years or older to be a team coach or captain.

Coach/Captain Acceptance form will be automatically sent after Team Registration has been submitted.

Student Acceptance form will be automatically sent after Coach/Captain Acceptance form has been submitted.

Mentor Interest form: To be filled out by someone interested in being a mentor. Please note that you must be 18 years or older to be a mentor. Please also see our information for mentors.
